Mentor Application

Thank for your interest in volunteering at Adult & Teen Challenge Northeastern Wisconsin. By completing this application, you are giving us permission to contact you regarding mentor opportunities. We also require your Social Security number & driver's license to perform a background check. Your information will not be used for anything else. You will also be contacted if there are any other documents needed. 

You will need the following information to complete your Mentor application

  1. Your contact information (e-mail, phone, address, etc)
  2. A character reference (name, e-mail and phone).
    1. Character references are usually done by a pastor, spiritual leader, or supervisor. They should know you well enough to have a good understanding of your character qualities including reliability and maturity. This Character Reference is especially important for those that would like to volunteer as a Mentor or work directly with our clients.
  3. Your SSN for a background check (as required by the State of Wisconsin).
    1. SSN’s are encrypted for security, submitted for a background check, and immediately destroyed. We do not store them in our database.  Please know that the State of Wisconsin will not allow volunteers with felonies to have direct, unsupervised, contact with our clients as in our mentoring program.
  4. Information on any dates on any treatment programs and/or criminal charges.

  5. NOTE:  For those Mentoring, driving or volunteering in a role where you take a client off-site, we also require your Driver’s License number.

    Mentors working directly with clients should have two (2) years of freedom from any chemical-related issues, treatment, or significant law and/or driving violations.
