Before You Begin Your Volunteer Application
Thank you for your willingness to serve with Adult & Teen Challenge Northeastern Wisconsin. By completing this application, you authorize us to contact you regarding volunteer opportunities and conduct a background check as required by law.
Please note:
Certain roles—especially those involving direct interaction with clients—require more in-depth screening to ensure the safety and integrity of our program. All personal information collected is kept confidential and used solely for these purposes.
What You’ll Need to Complete the Application:
- Basic Contact Information:
Your name, email, phone number, and mailing address. - Character Reference:
Please provide the name, email, and phone number of someone who can speak to your character. This should be a pastor, spiritual leader, employer, or supervisor—someone who knows you well and can speak to your reliability, maturity, and integrity.
A character reference is especially important if you’re applying to be a Mentor or work directly with our clients.
- Background Check:
A background check is required (per the State of Wisconsin). Your SSN is encrypted for submission, used only for the background check, and then securely destroyed. - Driver’s License Number:
Volunteers must have a valid driver’s license when transporting clients or taking clients off-site. - History of Treatment and/or Criminal Charges:
We ask for transparency regarding past substance abuse treatment or legal history.
Mentors and volunteers working directly with clients must have at least two (2) years of sobriety and be free from any recent legal or significant driving violations.
